A. Discharge Procedure
Step 1: Receive Discharge Notification
Patients eligible for discharge will be notified at least one day in advance to prepare.
Step 2: Complete Medical Records
After the doctor confirms the patient is stable for discharge, they will complete the medical records, provide instructions for follow-up appointments, and sign the discharge papers.
If the patient needs to continue medication after discharge, the doctor will prescribe and explain the medication usage in detail.
Patients who wish to take medication home should inform the nurse to notify the Pharmacy for preparation. Medication costs will be included in the hospital bill.
Step 3: Settle Hospital Fees
The patient or family member should go to the Billing Counter located in the main lobby to pay the hospital fees.
Some notes when paying:
Bring the hospital deposit receipt (if any)
Review the itemized cost list before payment
Pay the exact remaining balance after deducting the deposit
Keep the payment receipt for any future issues or support
Step 4: Complete Procedures and Receive Discharge Papers, Medication, and Related Documents
To complete the discharge procedure, please return the room key (if any), patient care cards, and any other hospital-owned items (clothes, blankets, mosquito nets, etc.).
The patient signs the discharge record to finalize the procedure, receives medication (at the Pharmacy), and obtains necessary documents such as the discharge paper (signed by the doctor), health insurance card, etc.
Step 5: Discharge
Please check and receive the following documents on the discharge day:
Prescription (if any)
Birth certificate (for childbirth cases)
Discharge paper signed by the treating doctor
Medical records, diagnostic imaging, and lab test results
Detailed treatment information (provided upon patient request)
Follow-up appointment slip if prescribed by the doctor
Personal belongings stored at the ward before admission or brought during hospitalization
B. Discharge Times
Discharge during Office Hours
Monday to Saturday:
Morning: 08:00 AM – 10:00 AM
Afternoon: 01:00 PM – 03:00 PM
Sunday: 08:00 AM – 10:00 AM
Discharge Outside Office Hours
Hospital staff will escort the patient to the cashier counter opposite the Emergency Room (A&E) located at the main lobby (ground floor) for payment and to complete discharge procedures.
C. Follow-up Appointment
Patients are kindly requested to follow the doctor’s scheduled follow-up appointment. If unable to attend, please contact Customer Service at 0974 508 479 for guidance.
Patients should come for an earlier follow-up if experiencing any of the following symptoms:
Surgical wound swelling, pain, abnormal discharge, or bleeding
Large swelling at the surgical site preventing movement
Fatigue, difficulty breathing, or blood in urine
For patients living far away, if the wound requires dressing changes, please visit the nearest medical facility for assistance to reduce the risk of infection from self-care at home.
